Ongoing skin issues

Hello All!
My handsome Rudy has had ongoing skin issues. I believe it started in January with a staph that he went on two rounds of antibiotics for and special shampoo. When this didn’t alleviate it we were sent to a doggy dermatologist who just confirmed it was staph. We had a more specialized antibiotic and a stronger shampoo. Still didn’t get rid of the itch. So then the derm said it must all be due to a good allergy. So I have Rudy on alligator protein. And over the past few days these black spots have appeared. I’m at a loss with what to do. Has anyone seen these? I plan on calling the vet tomorrow, but would love some ideas.

I’ve never seen anything like those spots before. Cheli gets black areas but it washes right off

Could it possibly be fleas? Is your dog on a flea and tick med?

Nope, not fleas. He’s been on an oral since January. The second time our regular vet put him on an antibiotic he also put him on Nexguard “in case it was mange.”

Then I have no idea, sorry. Some weird allergy to Nexgard ?? But won't hurt to add oral cetirizine antihistamine 1 mg/kg once daily ( I wouldn't go over 10 mg ). Also some probiotic since he's been on antibiotics eg. milk kefir, sauerkraut from refrigerator section of health food store, or commercial dog probiotic such as Probiotic Miracle, Proviable DC.
 
He hasn’t had his nexguard in almost a month and these spots just popped up a few days ago. The problem with adding the yogurt/kefir, etc is he is on a very strict diet as we try the novel protein to see if that helps his skin. I feel so bad for my poor little guy- getting him straightened out is proving to be difficult and sort of nightmarish. Every time I think we’ve made progress something pops up. Thank you for your advice though!
